Each family will receive a sheet containing 25 squares (Ballots) at $10 per square (you may take more than one sheet)

Each family will receive a sheet containing 25 squares (Ballots) at $10 per square (you may take more than one sheet)
· Write the name and contact info of the purchaser on the square they choose (use back of sheet for black squares)
After the $500 prize funds have been allocated (% of the # of sheets turned in): then the remaining monies will be split 80% (personal)/20% (team)
· ALL SHEETS NEED TO BE RETURNED TO JAMIE BY APRIL 26TH!
· PLEASE WRITE YOUR FAMILY NAME ON THE TOP OF YOUR SHEET TO ENSURE THAT WE ARE ABLE ALLOCATE FUNDS TO THE PROPER FAMILY FUNDRAISING ACCOUNT.
Jamie will be at CHP on Sat. April 6 between 5:30pm – 7:30pm with sheets to hand out OR refer to your last email to contact him to make pick up arrangements.
